The Welsh Ambulance Services NHS Trust provides a Non-Emergency Patient Transport Service (NEPTS) for people who are unable, for medical reasons, to make their own way to and from their hospital appointments. More details about eligibility are available: Am I Eligible For NEPTS Transport? - Welsh Ambulance Services NHS Trust

Older people can also access support through the travel costs scheme. More detail, including the eligibility criteria is available at: Get help with NHS travel costs | GOV.WALES.
