Ymhellach i'r ymatebion i WQ87336, WQ87337, a WQ87338, a wnaiff y Gweinidog gadarnhau a fydd y rhaglen sy'n ymchwilio i ddigwyddiadau diogelwch cleifion o COVID-19 nosocomiaidd yn ymchwilio i farwolaethau nosocomiaidd yn unig neu'r holl farwolaethau COVID-19 yn yr ysbyty?

The National Nosocomial COVID-19 Programme investigates all patient safety incidents of healthcare acquired nosocomial COVID-19 that occurred during the pandemic.

More broadly , mortality reviews form part of a standard process when someone dies in hospital in Wales. The reviews are designed to assess the care and treatment provided to a person and to identify any opportunities for learning and improvement. As part of the process, the reviews consider if there was anything that occurred during the person’s care that might have caused or contributed to their death.

Additionally, the medical examiner service has now been established to provide independent scrutiny of all deaths not investigated by the coroner.
