As referenced in my previous response, we are committed to better coordinating care and support for people at greater risk of needing urgent care, as well as supporting individuals to return home from hospital when ready. Ensuring that we learn from other relevant projects, such as the red bag scheme in England, will be a key facet of this work and we will be considering the outcome of the evaluation of the red bag scheme in England in development of service improvement in Wales.
