Delayed discharge data are already published monthly through the Welsh Government's Pathways of Care Delays reporting framework, including breakdowns by health board, local authority and recorded reason for delay. The framework provides health and social care partners with a comprehensive overview of the key causes of delays and supports local and regional action to improve safe and timely discharge. The data are reported by reason for delay rather than attributing responsibility to the NHS, social care or both, as delays are often the result of multiple system factors. Work towards resolving delay challenges is being taken forward between Welsh Government and regional partners using the published data to target improvement activity.
